65 yo and under. Clients can choose to stay on NDIS after passing 65 yo if they please, but new participants will only be accepted under that age. Plans are determined following initial assessment of needs and goals. Funds are allocated to categories such as support, equipment etc but can be shuffled if required and clinically justified. Plans are reviewed annually.
All quotes are submitted to and authorised by the NDIS. Approval and work orders accessed via NDIS supplier portal.
For people aged 65 and over.
1) CHSP: lower tier funding aimed at keeping older Australians safe and mobile in their home, typically distributed via HMMS. Eligibility determined by RAS, or clinical assessment in the case of Home Modifications. Used for recurring domestic services around the house such as gardening and cleaning, also one off lump sums for modifications such as bathrooms, ramps, grab rails, and stairlifts.
Generally funding will only cover part of the cost with the balance covered by the client. Financial agreement is between the client and builder. All quotes for CHSP must go to the Builder ONLY as they will compile cost of all works, add a service charge and issue quote to client. DO NOT discuss pricing with CHSP clients at any point, always refer them back to their builder.
2) HCP: Funding for higher needs clients. Eligibility determined by ACAT. 4 levels of packages available depending on degree of care required ranging from $9k - $52k per year. Distributed by Care co-ordination companies (of which there are hundreds).
Package values are set and allocation of funds determined by care coordinator depending on services required. Home Mods and AT are accessible but must be accounted for in allocation of funds which are accumulated over the year.
RAP is a department of the DVA and manage this federally funded scheme. Eligibility determined by military service, degree of impairment resulting from service, or be the widow/er of a service person. In order to receive subsidy for Home Modifications the participant must be a Gold card holder.
